Comparative performance of annual wellness visits between pharmacists and physicians at primary care clinics

Abstract
AbstractIntroductionAnnual wellness visits (AWVs) can help close care gaps, defined as differences between guideline‐recommended practice and actual care. The purpose of this study was to compare the performance of AWVs conducted by pharmacists and physicians toward care gap closure for the prevention and treatment of cardiovascular disease and osteoporosis.MethodsA retrospective chart review was conducted to evaluate interventions made during AWVs by pharmacists and physicians at primary care clinics. The variables of interest were (a) use of aspirin, statins, angiotensin‐converting enzyme inhibitors/angiotensin receptor II blockers, bisphosphonates, calcium/vitamin D, and proton pump inhibitors, and (b) need for dual energy x‐ray absorptiometry (DEXA) scans. The primary outcome was the rate of care gap closure examined between pharmacists' and physicians' panels.ResultsAmong 416 AWVs completed by pharmacists, 233 care gaps were identified, and 120 care gaps were closed. Similarly, among 169 AWVs completed by physicians, 103 care gaps were identified, and 27 gaps were closed. The rates of care gap closure were 0.29 ± 0.43 for physicians and 0.53 ± 0.48 for pharmacists (mean ± SD). The number of care gaps significantly improved between pre‐ and post‐AWVs for both groups (P < .01). The noninferiority test showed that the rates were noninferior for pharmacists compared with physicians (95% confidence interval: 0.12‐0.36). Overall, the most notable care gaps identified were (a) missing DEXA screenings, (b) inappropriate statin therapy, and (c) inappropriate calcium/vitamin D therapy. Reasons for not closing gaps were (a) declined drug therapy, (b) prior intolerance to medication, and (c) not completed DEXA scans. AWVs also corrected medication list inaccuracies and increased vaccination.ConclusionThis study showed that both pharmacists and physicians can utilize AWVs to close care gaps for the prevention and treatment of cardiovascular disease and osteoporosis, and the performance of pharmacists was noninferior to that of physicians.
